CAM-Colorado, LLC, 743 Horizon Court, Suite 201, Grand Junction, CO 81506, has submitted a
complete application for a technical revision (TR-03) to the Division of Reclamation, Mining
and Safety for its Fruita Loadout, mining permit No. C-2010-088. The proposed revision will
allow the site to use incremental bonding. Incremental bonding will allow a reclamation
bond adjustment. The loadout's current reclamation bond is $1,624,926. The loadout's
reclamation bond after adjustment will be $10,000.
The spur track and loadout are located south of State Highway 6 & 50, north of Interstate 70
and west of the old Fruita Refinery. The City of Fruita lies approximately 3.0 miles east of the
proposed spur track and loadout. Reed Wash runs through the central portion of the proposed
permit boundary. Loma Drain runs through the western edge of the permit boundary. The
proposed mine permit area is located on the U.S. Geological Survey 7.5 minute Mack
Quadrangle map within the following sections.

Section 3: Portions of the SE/4

The proposed Fruita Loadout permit boundary contains 208.6 acres of land that is all located on
private surface. CAM-Colorado, LLC owns 201.3 acres, the Union Pacific Railroad owns 5.3
acres and Fruita Development LLC owns 2.0 acres. The minerals within the proposed permit
area are owned by the respective private surface owners.
A copy of the technical revision application is available for public inspection at the Mesa County
Clerk and Recorder's office, 200 S Spruce St., Grand Junction, CO., and at the offices of the
Division of Reclamation, Mining & Safety, 1313 Sherman St., Room 215, Denver, CO, 80203, (303)
866-3567. Written comments, objections, or a request for an informal conference on the application
may be submitted to and additional information may be obtained from the Division of Reclamation,
Mining & Safety at the above address and must be received by the Division within 10 days after the
publication of this notice.

Petitioner, Redlands Mesa Metropolitan District (the "District"), provides this Notice of Dissolution, and states:
The District is a quasi-municipal corporation and political subdivision organized pursuant to the Special District Act, C.R.S. §§ 32-1-101, et seq. The District encompasses approximately 65.925 acres generally located northwest of the intersection of Monument Road and Mariposa Drive, City of Grand Junction, State of Colorado, legally described as follows:
Block 8 of Redlands Mesa Filing 1, according to the plat recorded at Reception No. 1957570, County of Mesa, Colorado.
Block 9 of Redlands Mesa Filing 1, according to the plat recorded at Reception No. 1957570, County of Mesa, Colorado.
Block 3 of Redlands Mesa Filing 1 Replat according to the plat recorded at Reception No. 2103247, County of Mesa, Colorado, EXCEPT that parcel conveyed to the City of Grand Junction in Book 2823 at Page 961 of the Mesa County records.

TO ALL INTERESTED PARTIES AND TO WHOM IT MAY CONCERN:
Pursuant to Rule 522.e.(2), the Colorado Oil and Gas Conservation Commission ("Commission" or "COGCC") Staff will apply to the Commission at its May 18-19, 2015 hearing for an Order Finding Violation ("OFV") against Petro Mex Resources ("Petro Mex") (Operator No. 10059). This Notice and Application is being served to adjudicate the allegations made in the below-described Notice of Alleged Violation ("NOAV"), the allegations of which are incorporated herein.
Petro Mex is an operator in Colorado and therefore must file a Form 8, Oil and Gas Conservation Levy, and pay the levy to the Commission. Petro Mex has failed to do this since 2007. On September 23, 2014, COGCC Staff issued NOAV No. 400691473 to Petro Mex for a violation of Rule 310 (COGCC Form 8, Oil and Gas Conservation Levy) of the COGCC Rules of Practice and Procedure, 2 CCR 404-1 ("Rule" or "Rules").
In May 2014, mail was returned from Petro Mex's address registered with the Commission and the operator did not file a corrected Form with the Commission upon Staff's request. On June 17, 2014, COGCC Staff issued NOAV No. 400628002 for a violation of Rule 302 (COGCC Form 1, Registration for Oil and Gas Operations)
Petro Mex is also the operator of the following Wells in Garfield and Mesa Counties.

c. Rule 603.f. Equipment, weeds, and trash); and
d. Rule 605.a. (Berms).
On September 10, 2014, COGCC Staff issued NOAV No. 200410334 to Petro Mex for violations at the Ismerelda 1-2-8-101 Well of the following Rules:
a. Rule 308 (COGCC Form 5/5A); and
b. Rule 326.b.(1) (Mechanical Integrity Tests, Shut-in Wells).
On September 12, 2014, COGCC Staff issued NOAV No. 200411870 to Petro Mex for violations at the Federal 1-15-8-101 Well of the following Rules:
a. Rule 308A & 308B (COGCC Form 5/5A);
b. Rule 319.b.(1) (Temporary Abandonment); and
c. Rule 326.b.(1) (Mechanical Integrity Test).
COGCC Staff requests that Petro Mex be found in violation of rules cited above and assessed a penalty pursuant to Rule 523.
NOTICE IS HEREBY GIVEN, pursuant to: 1) the general jurisdiction granted to the Oil and Gas Conservation Commission of the State of Colorado (Commission) under § 34-60-105 C.R.S.; 2) specific powers granted pursuant to § 34-60-106 C.R.S.; 3) the Colorado Administrative Procedures Act at § 24-4-105 C.R.S.; and 4) the Commission's Series 500 Rules at 2CCR 404-1, that the Commission has scheduled the above-entitled matter for hearing on:
