Team members will be responsible for working with health care providers in the coordination of medical care in a home setting. Home Health, hospice, acute care, and/or skilled nursing facility experience preferred. Requires current CNA certification, driver's license, auto insurance and reliable transportation. Job Type: Full-time Physical Therapy Assistant We are looking for an educated and licensed Physical Therapist Assistant to help patients improve their mobility and move forward. You will work closely and under the direction of the physical therapist, treating patients with medical problems or other conditions that limit their ability to move or perform functional activities.
